New Patients

New patients are asked to arrive 20 minutes early to complete paperwork. You can download, print, and complete several forms prior to your appointment by logging into our patient portal. Please bring any completed forms, a list of your medications, recent copies of your lab results and an insurance card to your office visit.

Billing

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards. If you have any billing questions, please contact our billing department at 303-298-1802.

You can also use our secure Online Bill Pay option to pay current statement balances online without setting up an account.

Insurance

We will submit claims to most major insurance carriers. Please bring with you your insurance card and an I.D. to every visit so we can ensure our records are accurate.

We participate with many insurance plans. We recommend that you call your insurance company to verify we participate with your plan and please make sure if your insurance requires a referral that you have it sent to our office prior to your visit.

Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And, please be prepared to present your insurance card at every visit.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise. If we are seriously delayed, we attempt to notify patients beforehand. If you are 15 minutes past your scheduled appointment time we will need to reschedule your appointment. Multiple no-shows will unfortunately result in termination.

Refill Requests

We are happy to help you with your prescription refill requests, but please call your pharmacy directly. They will send us your prescription information. If you have a concern or question about your medication, please contact one of our offices.
